Changing volume applies the gain you choose, whatever the source level was — useful when you know you want exactly +6 dB. Normalising measures the file first and then applies whatever gain brings its loudest peak to a target you set, which is what you want when matching several files to each other. Because you have run out of headroom. Digital audio has a hard ceiling, and once peaks reach it any further gain flattens the tops of the waveform, which is heard as harshness or crackle. Clip protection prevents the worst of it but cannot invent headroom. If a file needs a lot more perceived loudness, normalise first and then compress — do not simply keep adding gain.

Not with gain alone — gain raises signal and noise by exactly the same amount, so the hiss you can suddenly hear was always there, just below your threshold of hearing. Reduce the noise first with the Noise Reducer, then raise the level. Doing it in the other order makes the noise much harder to remove cleanly.
